This case continues to evaluate the case of Caitlynn, a 2-year old girl with confirmed cystic fibrosis (CF) and pneumonia at the Valley City Community Emergency room. The intention here is to present an evidence based care plan, a case contextualization, an interrogation of the treatment plan and an assessment of the remote interdisciplinary collaboration development concerning care outcomes.
Caitlynn is her name and she came to the emergency department with pneumonia, which is her third hospitalization due to this condition within the last six months. During her vital signs check, she showed signs of malnutrition, fever and increased respiratory rate, together with diminished breath sounds bilaterally and wheezing auscultated in the upper lung zones. The results of her sweat chloride test confirmed her diagnosis of cystic fibrosis.
Issues arose out of her relatives’ separation and limited resources where they typically lived. Skipping normal processes, Caitlyn’s main pediatrician Dr. Benjamin participated through Skype and texting. There is poor availability of advanced telemedicine tools in the region. Despite these concerns, remote teamwork was beneficial in the development of the care plan for Caitlynn.
Cystic fibrosis has a physiologic aspect that affects the normal secretory organs of the body, which leads to the accumulation of thick mucus, obstruction of airways, and organ damage (U.S. Department of Health and Human Services, n.d.). The interdisciplinary team developed and implemented an evidence based care plan that involved the following therapies.
The frequent administration of bronchodilators via nebulizer for the purposes of airway obstruction and therapeutic hypoventilation. Effectual incineration of the nebulizer is essential to avoid contamination.
The use of bronchodilator and corticosteroid therapy for treatment of inflammation and airway obstruction.
Supplementation of vitamins A, D, E, and K for addressing undernourishment and promoting growth. Additionally, iron, zinc, and calcium were emphasized for osteogenesis and immunity development (Stanford Children’s Health, n.d.).
Using the PET or practice question, Evidence, and Understanding approach, care plan was created applying the Johns Hopkins Evidence Based Practice Model. This model heading evidence also gives guidance for child care implementation and results (Springer Conveying, nd.). Through this lens Caitlynn’s care plan is designed to meet her specific needs as well as make sure that the practices are evidence based.
The collaboration at a distance posed two challenges and benefits. Telemedicine worked with exchange of information between the healthcare pack and Caitlynn’s family, which enabled care provision despite geographic cutoff points. There was however, different sets of challenges that limiting access to internet services and telemedicine devices posed. To tackle these problems, the emergency department team tried to modify existing technologies and build relationships with local communities to form networks.
Caitlynn’s case highlights the significance of evidence based practice in the interdisciplinary approach to the management of complicated conditions such as cystic fibrosis.The utilization of the Johns Hopkins model ensured a coordinated technique for managing her treatment, while remote collaboration allowed the care social event to overcome resource hindrances. Regardless of how challenges forge ahead, for instance, confined telemedicine access, the commitment to evidence-based practice works on consistent outcomes and supports critical lengths the board moves close.
